Effects of Indocyanine Green on Cultured Retinal Pigment Epithelial Cells

Abstract
This study was designed to investigate effects of indocyanine green (ICG) on cultured retinal pigment epithelial cells, used in macular hole surgery to improve visualization of the internal limiting membrane and facilitate complete dissection of the internal limiting membrane. After cultured porcine retinal pigment epithelial cells were exposed to ICG, the morphological change, toxicity and effect on cell proliferation was compared and analyzed with the control group exposed to balanced salt solution (BSS??). Cultured porcine retinal pigment epithelial cells were seeded and grown in 24-well and 96-well plates. Cells were exposed to BSS??, 0.1%, 0.5% and 1.0% concentration of ICG for 30 seconds, 1 minute, 5 minutes and 10 minutes. Cells were evaluated by phase-contrast microscopy, MTT assay or transmission electron microscopy. Retinal pigment epithelial cells exposed to ICG showed no morphological change except slight irregularity in cell size and demonstrated no toxicity and inhibitory effect on cell proliferation. But, among those exposed to over 0.5% concentration of ICG for over 5 minutes, retinal pigment epithelial cells proliferating to occupy the central defect area showed tendency to be transformed to fibroblast-like cells. Cytoplasmic vacuoles were observed in proliferating cells in ICG treated groups on transmission electron microscopy. These results suggest that ICG with a concentration of 1.0-5.0 mg/mL (0.1 - 0.5%) for about one minute exposure time appears to be safe to retinal pigment epithelial cells but loss of retinal pigment epithelial cells during macular hole surgery assisted by ICG may influences visual outcomes due to poor differentiation of proliferating cells.
